Starting a new depression treatment can bring a mix of emotions such as hope, uncertainty and even anxiety. If you are preparing for your first TMS session, knowing what to expect ahead of time can ease your nerves and help you feel more confident. Here, we will walk through what to expect with TMS therapy, from before your first session to post-treatment, so you can feel well-informed and empowered for your first appointment.

Before Your First Session

Before starting treatment, you will have already completed a thorough consultation with a provider to determine if TMS therapy is right for you. Once approved, you will schedule your first session.

How to prepare for your TMS appointment:

  • Eat and hydrate as you normally would. There is no need to avoid food.
  • Take medications as prescribed unless your doctor advises otherwise.
  • Wear comfortable clothing.
  • Avoid heavy hair products like gel or oils that might interfere with coil placement.
  • Bring headphones, water, or something to help you relax. Though these are not essential, it could be helpful to have something there to ease any nerves.

Your First Treatment Session

Because your TMS-certified physician needs to determine the most effective way to administer your treatment, your first session may last up to 90 minutes. You will be provided with protective earplugs, as the system produces a tapping sound. To begin, your TMS physician will perform a motor threshold test — a brief procedure that identifies the minimum magnetic field strength needed to cause a slight movement in your right thumb. This threshold helps determine the appropriate, personalized intensity level for your treatment.

Once the correct field strength is established, your TMS physician will identify the precise area on your scalp where treatment will be delivered, and the magnetic coil will be positioned accordingly to ensure optimal results. TMS therapy will then be administered over approximately 40 minutes. During this time, the device delivers rapid pulses of magnetic energy in 30-second intervals, which may feel like tapping on your scalp. While some patients find the tapping sensation uncomfortable, your physician can often make adjustments to improve your comfort during the session.

After Your Session

After each TMS treatment session, you can immediately return to your regular activities, including driving. Some patients may experience mild side effects such as a headache or scalp discomfort, which are typically temporary and tend to improve with continued treatment. Over-the-counter pain relievers can help manage this discomfort if needed. If the side effects persist, your TMS physician may adjust the magnetic field strength to enhance your comfort during sessions.

In clinical trials, most patients who responded to NeuroStar TMS Therapy began noticing improvements by the fourth week of treatment. However, response time can vary — some may experience benefits sooner, while others may need more time. It is important to communicate regularly with your physician about your depression symptoms throughout the course of therapy.

Building a Routine

TMS is most effective when done consistently. You will likely be scheduled for 5 sessions per week for 4-6 weeks. Over time, many patients report reduced depressive symptoms and mood improvements.
